[NUUG fiksgatami] OSM related commits broken map clicking for fixmystreet

Hi,

Sorry about that; the lat/lon hidden fields were being output twice on
the report form page, and in trying to fix that I ended up having it
outputting neither on the first map page instead. Should be okay now.
